![]() The Province requires that residents of Toronto without OHIP numbers requesting vaccination receipts have a COVID ID number, which is issued by Toronto Public Health and several of its community partner agencies. A health card is not needed, however individuals must bring an acceptable form of identification which may include government-issued ID (including non-Canadian and expired documents) such as a driver’s licence or passport, a piece of mail with your name on it, a pay stub or a student card.
